چکیده

Introduction: Percutaneous approach to the kidney is a very useful alternative in renal surgery which minimizes the morbidity of a variety of procedures. Objectives: To evaluate the efficacy, practicity of a reusable system of metallic coaxial dilator coupled to a special puncture needle to perform lumbar puncture and dilation of the percutaneous tract. Methods: A randomized experimental study was carried out involving 50 individuals. These patients were randomized as follows: Group 1 had the procedure performed with the conventional disposable Amplatz set traditionally used for this procedure. Patients in Group 2 were operated utilizing the new reusable coaxial set specially designed for this purpose. The following parameters were measured to compare the 2 groups: sex Corporeal Mass Index (CMI), Pre and postoperative hematocrit and hemoglobin. Time interval between the puncture, dilation and access to the renal pelvis and insertion of the nephroscope were also analyzed. Results: There was no statistical difference between the 2 groups concerning hematocrit and hemoglobin changes when comparing prewith postoperative period. Patients in Group 2 required a significant lower time between puncture and final access to the upper collecting system. Conclusion: The new coaxial dilator set showed to be as safe as the conventional Amplatz set with the advantage of reusability and decrease of tract dilation time.

A novel percutaneous nephrolithotomy (PCNL) set: The ‘Economical One-shot PCNL Set’ (Ecoset)

OBJECTIVES To suggest a novel disposable percutaneous nephrolithotomy (PCNL) set that we named the 'Economical One-shot PCNL Set' (Ecoset), which consists of a single 30-F dilator, 30-F sheath, and 8-F polyurethane dilator, as use of a 'one-shot' dilatation technique during PCNL is becoming widespread. PATIENT AND METHODS The medical records of 42 patients with kidney stones who had undergone...

Collecting system percutaneous access using real-time tracking sensors: first pig model in vivo experience.

PURPOSE Precise needle puncture of the renal collecting system is an essential but challenging step for successful percutaneous nephrolithotomy. We evaluated the efficiency of a new real-time electromagnetic tracking system for in vivo kidney puncture. MATERIALS AND METHODS Six anesthetized female pigs underwent ureterorenoscopy to place a catheter with an electromagnetic tracking sensor into...

ERCUTANEOUS nephrolithotomy (PCNL) is a surgical procedure to remove stones from the kidney through a small puncture wound through the skin after intracorporal lithotripsy. PCNL

Percutaneous nephrolithotomy (PCNL) for the treatment of renal stones and other related renal diseases has proved its efficacy and has stood the test of time compared with open surgical methods and extracorporal shock wave lithotripsy. However, access to the collecting system of the kidney is not easy because the available intra-operative image modalities only provide a two dimensional view of ...

Needle Force Sensor, Robust and Sensitive Detection of the Instant of Needle Puncture

A force sensor based method to detect the instant of needle puncture to the layers of tissues is proposed. A set of needle and sheath attached to a pair of coaxial force sensors was used to separately measure the cutting force at the needle tip and the sidewall friction. Change of the force profile was observed and marked at the instant of the puncture, and robustly eliminated artifacts caused ...

Kidney Access Device

INTRODUCTION Percutaneous nephrolithotomy is the most complicated stone surgery technique to learn. The steep learning curve is related mainly to obtaining precise renal access by puncturing the targeted calyx. A minimally misaligned puncture may lead to torrential bleeding, failure of the surgery, and complications. Renal puncture can take a long time, and the increased fluoroscopic time is a ...
